Your policy number consists of the 5 to 6 digits in the upper right corner of your billing statement or dec page.
Below is an example with the policy number highlighted in yellow:

Your activation code is a six digit number that can be found on your declaration page or billing statement.
On the billing statement, look to the right of the bold header Notice of Payment Due. Then look above Date Prepared. You should see the activation code printed there.
On the declaration page, you'll find your activation code underneath your loss deductible near the top right portion of the document.
